Nurse’s Corner

Dear Mitchell Elementary School Families,
I look forward to my role as school nurse in HoustonlSD. Please come to visit me in the
Mitchell Health Office. I would very much like to meet you and introduce myself to you and
your children. The information provided is general information pertaining to the daily
operation of the nurse's office.
Office hours: 7:30 am until 2:50 pm Tuesday - Friday.
The office is located inside the main office.

Please contact the school nurse when:
- Contagious conditions such as chicken pox, flu, strep throat, whooping cough, pneumonia, conjunctivitis or head lice, your child is absent for > 3 days due to physical illness, injury or mental health condition.
- Pre-existing medical conditions.
- There is a new medical diagnosis or change in your child’s physical or emotional health status, including a new allergy.
- A newly prescribed medication or a change in dose of a current medication.
- A serious injury, illness or condition requiring emergency treatment, hospitalization or surgery.
- A head injury or concussion.
- An injury requiring sutures, casting, splints, crutches or wheelchair.
- There's a transitional change in your family or traumatic event that may affect your child.
Immunization Information
All immunizations must be current. If your child's immunizations are incomplete, you will be sent a letter by the nurse to inform you. In order for your child to stay in school the immunizations required must be obtained and proof provided to the nurse within the designated time.
Medication Policy
- Health Services requests that medication be administered by a parent/guardian before and after school hours if possible. Medication that is to be given twice a day should be given before school and at bedtime. Medication that is to be given three times a day is generally given at home before school, after school and at bedtime.
- ALL medications including over the counter medications require documentation of an order by a licensed prescriber and consent by parent or guardian. Administration of Medication during School Hours form must be completed by a PHYSICIAN. (Forms are found in the school clinic and on the HoustonISD web site.)
- New medication orders are required at the start of each new school year.
- Medications must be delivered and picked up by the parent/guardian or designated adult to the school nurse/designated staff in a pharmacy or manufacturers labeled medication container.
- Medications are accepted in original labeled containers only. No pill boxes, envelopes, plastic bags...
- Students are not permitted to bring medications (including over the counter medications) to school except for those medications that have been approved to carry or self-administer.
- All medication orders expire at the end of each school year and medications must be picked up by a parent/guardian by the time of dismissal at the close of the school year. This includes students attending summer school.
- ALL MEDICATIONS THAT ARE NOT PICKED UP BY DISMISSAL AT THE END OF THE SCHOOL YEAR WILL BE DESTROYED.
Sending Students Home
The student will be sent home for the following conditions:
- A temperature of 100 degrees or higher.
- A rash/skin eruption, with or without drainage of unknown origin that has not been diagnosed by a doctor as being non-contagious. Requires a medical release for the student to return to school.
- Active vomiting or diarrhea.
- Suspected contagious illness. Requires a medical release for the student to return to school.
- Serious injury or appears obviously ill.
